School Supply List and School CalendarPhysical Forms All new students and Kindergarten students and students entering sixth grade are required by the state of Illinois to have an updated physical form on file at the school. This form should show that all immunizations are up to date and that your child is able to participate in school. This is a standard state form that you can get from the doctor’s office by calling them and letting them know your child will attend school in the fall. Returning students entering Kindergarten or students that have had new immunizations since last year’s physical form should bring an updated copy. ** New for the 2012-2013 school year - Students entering 6th and 9th grade must have proof of the Tdap immunization. Please see your child's doctor for more information. ALL primary and toddler students also need an updated physical/immunization report on file. Vision Screening (for students age 5 and over) Public Act 95-671, effective January 1, 2008, requires that all children enrolling in kindergarten in a public, private or parochial school and any student enrolling for the first time in a public, private or parochial school shall have an eye examination.
Dental Form All Illinois children in kindergarten and Grades 2 and 6 are required to have an oral health examination. • This requirement includes all public, private, and parochial schools. • Examinations must be performed by a licensed dentist, and he/she must sign the proof of school dental examination form. • Each child is required to present proof of examination by a dentist prior to May 15 of the school year. • School dental examinations must have been completed within the 18 months prior to the May 15 deadline.
2011 - 2012 Supply List - NEW SUPPLY LISTS Toddler 2011 1 pair of simple sandals/shoes to leave at school - Light weight, easy on and off - no croc style shoes please 3 complete outfits (shirt, pants, socks, and underclothing) label all items with the child's name diapers if applicable 1 package of baby wipes 1 small backpack Primary 3-6 Year Olds - 2011 1 pair of slippers or indoor shoes - no big head or furry characters please – soles should be non skid (no croc style shoes) – and slippers should go around the ankle (no open heels) - Full Day Students need a lunch box and thermos for hot meals (soup, macaroni and cheese, etc.) We will discuss the best options at Parent Orientation - there is no need to get a lunch box before that evening. One pair of rubber pull-on boots to be left at school (full day students only) At Parent Orientation (or the first week of school) your family will be assigned a few other items to bring in as a community classroom supply.
Elementary Students - 2011 All elementary students: (Most supplies are community supplies with the exception of page protectors and binders. Please label the binders and page protectors with the student's name.) #2 Pencil Set - 48 pencils total - no novelty pencils please as the leads break easily 1 large pink or white eraser 1 package of colored markers 1 packages of colored pencils 2 boxes of tissues 1 package of pencil top erasers 1 box of 24 crayons 1 hand held pencil sharpener with shaving catcher 1 tray of water color paints box of 100 page protectors First year students: one (1 1/2 inch) 3 ring binder with a cover insert AND a composition notebook - please label only the binder and page protectors with your child's name Second and third year students: one (2 inch) 3 ring binder with a cover insert - please label only the binder and page protectors with your child's name Fourth grade and above: one (2 inch) 3 ring binder with a cover insert and one (1 1/2 inch) binder with 5 or more dividers AND (1) two pocket folder AND one composition notebook - please label only the binders and page protectors with your child's name
