School Supply List and School CalendarPhysical Forms All new students and Kindergarten students and students entering sixth grade are required by the state of Illinois to have an updated physical form on file at the school. This form should show that all immunizations are up to date and that your child is able to participate in school. This is a standard state form that you can get from the doctor’s office by calling them and letting them know your child will attend school in the fall. Returning students entering Kindergarten or students that have had new immunizations since last year’s physical form should bring an updated copy. Vision Screening Public Act 95-671, effective January 1, 2008, requires that all children enrolling in kindergarten in a public, private or parochial school and any student enrolling for the first time in a public, private or parochial school shall have an eye examination.
Dental Form All Illinois children in kindergarten and Grades 2 and 6 are required to have an oral health examination. • This requirement includes all public, private, and parochial schools. • Examinations must be performed by a licensed dentist, and he/she must sign the proof of school dental examination form. • Each child is required to present proof of examination by a dentist prior to May 15 of the school year. • School dental examinations must have been completed within the 18 months prior to the May 15 deadline.
